Before You Start
- Please have all personal information ready, including full registered GP surgery details and 1 form of ID
- Residency: You must be currently UK-based to proceed with this application.
- Payment is taken at the end of the form of only £72.99 with no hidden fees. We cannot proceed with your application until payment has been made in full
- Medical Declaration: You must be ready to consent to allowing Vortex Medicals to access your full medical records. You MUST fill in and attach the consent form which can be found below to download. (please note GP surgeries require a wet signature)

2. Medical Consent & Consent form Upload
Upload your completed GP Subject access request form which can be found here and consent to us accessing your records. You must also upload 1 form of ID & proof of address so we can confirm it is the applicant making the application and reduce delays from the GP surgery. If you prefer to make a paper application then please contact us.
3. Payment & Submission
Process your payment and submit the form to the team then sit back, relax and leave the rest to us.

What Happens Next
Your application has been received and is now being processed. Our clinical team aims to review and finalise all firearms medical reports within 5 to 7 working days from receiving your medical records from your GP. Please stay reachable, as one of our GMC-registered doctors may contact you for clarification regarding your medical history. Once finalised, the completed report will be forwarded to you and where appropriate your police force, ensuring a seamless continuation of your certificate application.
